CLEANING SERVICE CONTRACTS

GENERAL:
PHC Cleaners LLC agrees to provide all labor, supervision, material, and equipment necessary to assure performance of specified cleaning service at your residence. This shall include all services described in the written specifications. PHC Cleaners LLC agrees to furnish such cleaning services for length of contract or until closed or cancelled by either party.

SPECIAL SERVICES:
Carpet cleaning is available - Price quoted upon request.
Waxable hard surface floors can be stripped and refinished or scrubbed and re-waxed - Price quoted upon request.
Windows - $4.00 per window

SERVICE SCHEDULE/RESCHEDULING POLICY
Cleaning service operations described in this comprehensive program will be performed for the duration of your specific terms.
The cleaning crew will observe holidays observed by the customer. PHC Cleaners LLC is prepared to adapt this work schedule to coincide with the needs and requests of the customer provided that such requests do not alter the cost of operations.
The customer or the representative of the customer is expected to contact PHC Cleaners LLC within 48 hours of cleaning cancellation or rescheduling of a prior scheduled cleaning service. If PHC Cleaners LLC is not contacted or informed of such cancellation or rescheduling within 48 hours, the customer will be charged the cost of that specific cleaning.

HOLIDAYS
Holidays as follows will be considered by PHC Cleaners LLC:
- Thanksgiving
- Christmas Eve (Closed after 2 pm)
-Christmas Day
-New Years Eve (Closed after 2 pm)
-New Years Day
Holidays are subject to change based on client’s schedules and can alter the holiday Schedule of PHC Cleaners LLC.

INVOICING/LATE FEES
All invoicing will be itemized according to monthly work or for special tasks. Invoicing will be on the 1st of each month. Payment policy is net 15 days of receiving invoice.
If the monthly payment is not received by the 25th day of the of following month for cleaning services rendered, there will be a 10% late fee charged to the invoice. Every 10 days following an additional 10% late fee charge will be assessed to the invoice.

Example: Services are rendered for the month of January. Invoices for services rendered in January are delivered on the 1st of Febuary the amount due ($100). All invoice payments are due Febuary 15th. Any over due payments received at Febuary 25th will be charged with a 10% late fee, total amount ($100+$10=$110.00). Every ten days following a 10% late fee will be assessed to the invoice until payment is complete.

SUPERVISION
Adequate personnel and supervision will be furnished to ensure quality service.

SUPPLIES
The customer will furnish all consumable products inclusive of but not limited to: toilet tissue, towels, trash liners and hand soap. If desired, PHC Cleaners LLC can provide these products and invoice them separately. PHC Cleaners LLC will furnish all cleaning supplies inclusive of but not limited to: cleaning agents, disinfectants, etc.

EQUIPMENT
PHC Cleaners LLC will furnish and maintain all necessary cleaning equipment inclusive of but not limited to: floor machines, buffers, carpet extractor, vacuums, maid carts, mop buckets, wringers, mops and brooms.
PHC Cleaners LLC will comply with current OSHA regulations and proven procedures pertaining to all work performed at the customer's location.

INSURANCE
PHC Cleaners LLC will furnish all forms of insurance required by law and shall maintain the same in force.

Personnel supplied by PHC Cleaners LLC are deemed employees of PHC Cleaners LLC and will not for any purpose be considered employees or agents of the customer.

The term of this agreement shall be for a period of one (1) year and shall automatically renew for additional one (1) year periods on the anniversary date of this agreement. Unless otherwise notified by customer.

CANCELLATION
This agreement may be terminated or canceled at any time with a minimum of thirty (30) days written notice from either party.

If the monthly payment is not received by the 25th day of the of following month for cleaning services rendered, there will be a 10% late fee charged to the invoice. Every 10 days following an additional 10% late fee charge will be assessed to the invoice.

Example: Services are rendered for the month of January. Invoices for services rendered in January are delivered on the 1st of Febuary the amount due ($100). All invoice payments are due Febuary 15th. Any over due payments received at Febuary 25th will be charged with a 10% late fee, total amount ($100+$10=$110.00). Every ten days following a 10% late fee will be assessed to the invoice until payment is complete.

SUPERVISION
Adequate personnel and supervision will be furnished to ensure quality service.

SUPPLIES
The customer will furnish all consumable products inclusive of but not limited to: toilet tissue, towels, trash liners and hand soap. If desired, PHC Cleaners LLC can provide these products and invoice them separately. PHC Cleaners LLC will furnish all cleaning supplies inclusive of but not limited to: cleaning agents, disinfectants, etc.

EQUIPMENT
PHC Cleaners LLC will furnish and maintain all necessary cleaning equipment inclusive of but not limited to: floor machines, buffers, carpet extractor, vacuums, maid carts, mop buckets, wringers, mops and brooms.
PHC Cleaners LLC will comply with current OSHA regulations and proven procedures pertaining to all work performed at the customer's location.

INSURANCE
PHC Cleaners LLC will furnish all forms of insurance required by law and shall maintain the same in force.

TERM
The term of this agreement shall be for a period of one (1) year and shall automatically renew for additional one (1) year periods on the anniversary date of this agreement. Unless otherwise notified by customer.

CANCELLATION
This agreement may be terminated or canceled at any time with a minimum of thirty (30) days written notice from either party.

GENERAL:
PHC Cleaners LLC agrees to provide all labor, supervision, material, and equipment necessary to assure performance of specified cleaning service at your residence. This shall include all services described in the written specifications. PHC Cleaners LLC agrees to furnish such cleaning services for length of contract or until closed or cancelled by either party.

SPECIAL SERVICES:
Carpet cleaning is available - Price quoted upon request.
Waxable hard surface floors can be stripped and refinished or scrubbed and re-waxed - Price quoted upon request.
Windows - $4.00 per window

SERVICE SCHEDULE/RESCHEDULING POLICY
Cleaning service operations described in this comprehensive program will be performed for the duration of your specific terms.
The cleaning crew will observe holidays observed by the customer. PHC Cleaners LLC is prepared to adapt this work schedule to coincide with the needs and requests of the customer provided that such requests do not alter the cost of operations.
The customer or the representative of the customer is expected to contact PHC Cleaners LLC within 48 hours of cleaning cancellation or rescheduling of a prior scheduled cleaning service. If PHC Cleaners LLC is not contacted or informed of such cancellation or rescheduling within 48 hours, the customer will be charged the cost of that specific cleaning.

HOLIDAYS
Holidays as follows will be considered by PHC Cleaners LLC:
- Thanksgiving
- Christmas Eve (Closed after 2 pm)
-Christmas Day
-New Years Eve (Closed after 2 pm)
-New Years Day
Holidays are subject to change based on client’s schedules and can alter the holiday Schedule of PHC Cleaners LLC.
